The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus‑based direct services and supports at 13 centers located throughout the state.

Essential Job Functions
  • Facilitates community placements for individuals residing at SGSSLC into the community programs. Plans and develops the individuals’ Community Living/Discharge Plan and monitors activities. Coordinates group home visits for individuals and assists with transportation state‑wide.
  • Maintains facility admission data; coordinates Chapter 55 and Article 46B commitments for forensic evaluations.
  • In the absence of the Admissions Director, chairs the Admissions and Transition Committee to determine appropriateness of services for admission requests. Participates on the Plan of Improvement group or Quality improvement Committee (QIC) to develop and monitor the facility’s Plan of Improvement.
  • Serves as co‑lead for provision T of the Department of Justice Settlement Agreement.
  • Coordinates the facility practice of the Community Living Options Information Process with the Local Authority. Coordinates staff training on community living options and oversees the provider fair and other community education activities for individuals and staff.
  • Assists IDTs with the temporary consignment of state hospital psychiatric hospitalizations.
  • Attends required meetings, participates in committee functions, and completes required reports and other assignments as provided/requested.
  • Works cooperatively and communicates effectively with other departments as related to the care and treatment of individuals served.

Other duties as assigned, including but not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.
