Physical Therapist Home Health

Join Jet Health, Inc.'s Trio Home Health program in Flower Mound, Lewisville, TX. We are seeking licensed Physical Therapists for full‑time, part‑time, or PRN/Per Diem shifts to provide PT services to patients in the home health program.

Responsibilities
  • Evaluate patients' physical and functional abilities to develop individualized care plans.
  • Plan, implement, and modify physical therapy treatment programs based on patients' medical referrals and assessments.
  • Provide hands‑on therapy, exercises, and techniques to help patients improve mobility, relieve pain, and restore function.
  • Educate patients and families on therapeutic exercises, proper body mechanics, and injury prevention.
  • Collaborate with interdisciplinary healthcare teams for coordinated and holistic patient care.
  • Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans.
  • Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy.
Qualifications
  • Graduate of an accredited School of Physical Therapy.
  • Current state license as a Physical Therapist, or valid multi‑state license under the National Physical Therapy Compact.
  • Minimum of two years experience in a clinical care setting, preferred Home Health, geriatrics, or medical‑surgical.
  • Excellent interpersonal and communication skills.
  • Strong nursing skills as defined by generally accepted standards.
  • Reliable transportation and valid auto liability insurance.
  • Ability to work flexible hours and travel locally as needed.
  • Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
