Job Summary

This senior‑level coordinator performs advanced consultative services and technical assistance for individuals with intellectual disabilities as they transition from a State Supported Living Center into community settings. Responsibilities include coordinating educational opportunities, identifying obstacles to community transition, developing action plans, and may involve supervisory duties. Work is carried out under limited supervision with considerable latitude for initiative and independent judgment.

Essential Job Functions
  • Facilitates community placements and develops the individual’s Community Living/Discharge Plan.
  • Co‑ordinates group home visits and assists with state‑wide transportation.
  • Maintains facility admission data and coordinates Chapter 55 and Article 46B commitments for forensic evaluations.
  • In the absence of the Admissions Director, chairs the Admissions and Transition Committee.
  • Participates on the Plan of Improvement group or Quality Improvement Committee to develop and monitor the facility’s Plan of Improvement.
  • Serves as co‑lead for the provision of the Department of Justice Settlement Agreement.
  • Coordinates practice of the Community Living Options Information Process with the Local Authority.
  • Assess staff training needs and oversees provider education activities.
  • Assists IDT’s with the temporary consignment of state hospital psychiatric hospitalizations.
  • Attends required meetings, completes reports, and performs other assignments as directed.
  • Communicates effectively with other departments related to the care and treatment of individuals served.
Knowledge,

Skills and Abilities
  • In-depth knowledge of community based services available to individuals.
  • Proficiency in social services work and related laws and regulations.
  • Skill to collect and analyze information to develop community living plans.
  • Ability to develop and evaluate administrative policies, prepare concise reports, and facilitate meetings.
  • Excellent organizational, time‑management, and communication skills.
  • Computational skills with Word, Excel, and Outlook.
  • Commitment to facility mission and workplace/service values.
  • Ability to provide training to staff, volunteers, and individuals.
  • Agility in travel as needed and maintain compliance with attendance and dependability guidelines.
Qualifications
  • Experience in a field relating to human services.
  • Graduation from an accredited four‑year college or university with major coursework in a human services field.
  • Work experience related to the duties of the position.
  • Applicants must pass a fingerprint criminal background check, pre‑employment drug screen, and registry checks including CANRS, Nurse Aide, Medication Aide, and HHS List of Excluded Individuals/Entities (LEIE).
  • Males aged 18–25 must register with the Selective Service.
  • All State Supported Living Center employees are subject to Random Drug Testing.
  • Flexibility in work hours may be required, including overtime, extended hours, weekends, and holidays.
  • All applicants must be at least 18 years of age.
Additional Information
