Organization Context

The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The centers provide campus-based direct services and supports at 13 centers located throughout the state. Learn About Our State Supported Living Centers.

Responsibilities (Essential Functions and Related Duties)
  • The QIDP II is central to the overall responsiveness and effectiveness of an individual's active treatment program at an SSLC through oversight of the Individual Support Plan (ISP) process, centered on the individual's preferences, strengths, and needs, and includes the creation of the ISP.
  • Coordinate, monitor, document, and follow-up on all programs, services, and plans for individuals on the caseload. Lead and coordinate the interdisciplinary team (IDT) and monitor the active treatment program.
  • Determine the need for program revision; identify and resolve training inconsistencies; assure timely follow-up for IDT recommendations.
  • Facilitate acquisition of skills and adaptive behaviors; promote competent staff interactions in program implementation and behavior management.
  • Comply with federal and state standards governing ICF/IID, the Settlement Agreement with DOJ, and SSLC policies and procedures.
  • Work with individuals who may have complex needs and may exhibit aggressive and/or self-injurious behaviors.
  • Demonstrate strong organizational skills, balance competing priorities, and work a flexible schedule to ensure effective oversight.
  • Demonstrate strong computer skills and use of electronic records systems for tracking and documentation.
  • Work under limited supervision with latitude for initiative and independent judgment.
Knowledge,

Skills and Abilities

(KSAs)
  • Knowledge of the IDT process and people-centered planning.
  • Knowledge of rehabilitation and skills acquisition programs for persons with intellectual disabilities, including program design and data analysis.
  • Knowledge of PBSPs and PNMPs or ability to develop knowledge of discipline-specific plans and processes.
  • Ability to coordinate programmatic and clinical services and supports; prepare and maintain records; communicate clearly in speech and writing.
  • Ability to assess and solve problems; work independently and prioritize work; adapt to change.
  • Ability to work harmoniously and professionally with staff at all levels and with LARs; perform direct care work as needed, including emergency procedures.
  • Skills in facilitating IDT meetings and using a PC to create documents/reports; experience with electronic records systems or databases.
