Occupational Therapist, VNA of Cape Cod – Cape Cod Healthcare
Join our team to provide comprehensive occupational therapy services for patients referred to our Visiting Nurse Association. 37.5 hours per week, Monday–Friday, 8am–4pm, with rotating weekends and holidays.
Location:
Lower/Outer Cape.
- Maintainuffle annual HHRGAnd National Standards as appropriate.
- Use HHRG values to establish visit frequency and interdisciplinary utilization.
- Ensure annual outcomes meet National Benchmarks.
- Refer to community resources as appropriate.
- Evaluate clinical needs, initiate, develop, implement and revise patient plans of care, addressing all problems and documenting accordingly.
- Participate in interdisciplinary patient case conferences and evaluate plan effectiveness.

DK a Home Health Aide plan of care every 14 days, providing documented supervision. - Notify MD/other involved care providers of significant changes in the patient’s condition.
- Complete admission and other paperwork timely.
- Document narrative care: skillful care, clear concise language, documenting teaching, direct care, and patient/caregiver responses.
- Obtain and clarify valid MD orders for all delivered care.
- Plan and coordinate discharges, inform patient/MD, and record disposition in the discharge summary, completing all paperwork.
- Discuss with COTA, provide written baseline data, treatment plan, status and precautions prior to initial COTA visit; delegate care, supervise, and case‑conference per guidelines archivos‑dissed.
- Teach patient family, significant other in the occupational therapy program.
- Perform all other duties as assigned.
- Bachelor of Science or Master's in Occupational Therapy from an accredited institution.
- Current OT license in Massachusetts.
- Minimum 1 year clinical experience and CPR certification.
