Overview
To serve as the company’s scheduling coordinator of the patient visits by providers, nurses, and medical assistants.
Reporting RelationshipSupervised by:
Practice Manager
Interrelationships:
Care team members, contracted facilities, care partners, patients, patient families, and other staff members.
- Schedule visits of all patients on Dynamic Wound Care’s patient roster.
- Have a good understanding of care partners preferences for schedule of visits.
- Communicate with care providers on finalization of schedule and any changes made in a timely manner.
- Have a good understanding of patients’ needs and take into consideration when creating the visit schedule.
- Daily calls to confirm the next days’ visit for each patient and documenting in the patient’s chart.
- Observes confidentiality and safeguards all patient related information.
- Accepts responsibility for regular attendance and punctuality and fulfills job requirements during scheduled working hours.
- Communicate with field staff on any changes (day-of) to the schedule.
- Field incoming calls regarding questions or concerns pertaining to the visit schedule of patients.
- Track patient status, communicating known hospitalizations, deaths, travel etc. to the clinical team.
- Assist in answering phones in the office when needed.
- Update and maintain the patient master list.
- Update and maintain the patient hospitalization list.
- Call the hospital list of patients to see if they have been discharged or know of date of discharge.
- Work the referral log and place on the schedule promptly.
- Send weekly schedules to facilities, home health, via fax, text message, or email – to be completed by Thursday at 3pm in order to make any necessary changes or corrections.
Education/Degree
High school diploma or GED equivalent
- Knowledge/Skills/Ability
Ability to work independently, make accurate and at times, quick judgements. Ability to respond timely and appropriately to requests pertaining to the day-to-day schedule of patient visits. Acceptance of and adaptability to different social, racial, cultural, and religious modes. - Experience
Minimum of 2 (two) years of experience in scheduling in a Healthcare setting required. Understanding of medical terminology, ability to multi-task, coordination of schedules with other agencies and facilities. Computer skills, customer service skills, ability to meet deadlines timely and ability to make last-minute necessary corrections or changes upon request.
- Must possess sight/hearing senses or use appropriate adaptive devices that will enable senses to function at a level required to meet the essential duties of the position.
- Must be able to sit for long periods of time.
- Use of phone, keyboard, mouse, computer screens.
- Must be able to work independently, make judgements based on assessments and data available and act accordingly.
- Must be flexible, innovative and possess good interpersonal skills.
- Must be able to cope with mental and emotional stress and demonstrate emotional stability.
Must have a current valid driver’s license, auto liability insurance, and reliable transportation.
