Overview

Job Responsibilities
  • Assesses for immediate and long-term service needs.
  • Applies the Housing First approach to serving people with housing and other service needs.
  • Screens individuals for eligibility for services and/or benefits and assists as needed.
  • Assists an individual with goal-setting and identifies barriers to services, seeking to eliminate them where possible.
  • Facilitates linkages to services and interfaces with external service agencies.
  • Assists with applications for various services/resources and helps facilitate transportation to required service appointments.
  • Engages high-risk individuals (due to suicidal or homicidal thoughts/behaviors, history of hospitalizations, and/or history of addiction or risky substance use) with a focus on linking them to needed higher levels of care.
  • Participates in clinical staffing and other clinical team meetings to provide updates on care and coordinate care with other disciplines.
  • Utilizes consumer care resource funding to help people meet needs related to the treatment plan.
  • Works with program clinicians and engages individuals in developing and implementing discharge plans to ensure service connections are in place.
  • Networks with community service organizations to create partnerships for discharge planning.
  • Engages the highest-risk individuals in wellness checks for at least one year after the initial intervention.
  • Collects and reports outcome data as part of routine wellness checks with service recipients.
Accountabilities

Provision for Case Managers Services – Work with participants and identified support systems to set long-range and short-range goals, ensuring professional recommendations are incorporated into goals and outcomes. Ensure delivery of services meets established intensity and frequency as clinically indicated.

  • Ensure documentation and required reports are completed within timelines as required by funding source, policy, and supervisor.
  • Makes referrals to appropriate medical, psychiatric, education, community resources and other social services agencies and/or providers in a timely manner to address identified needs.
  • Document and follow up timely to ensure services are in place and appropriate.
  • Establish and maintain ongoing rapport with participants and their identified support systems to assist participants in achieving identified goals.
  • Establish and maintain rapport with referral sources and other community agencies to coordinate services for participants.
  • Respond to referrals and requests for services within timelines as required by funding source, policy, and supervisor.
  • Empower and encourage participants and their identified support systems to be active partners in services.
  • Ensure all participants and their significant others are treated with respect and dignity; ensure their rights are preserved.
  • Maintain a minimum 40-hour work week and participate in case reviews, team meetings, staff meetings and other required trainings.
Client File Maintenance
  • Records, plans, legal forms, and other documentation are kept accurate and up to date.
  • All services provided are documented and appropriately signed and dated in the correct location in the chart based on funding source, policy, and supervisor.
  • Documentation reflects current services, interventions, participant participation, progress towards goals and any challenges or barriers to achieving goals.
Staff Development
  • Participate in required training activities and programs.
  • Participate in trainings for certifications, credentials or specialties as determined appropriate by the organization, funding source or supervisor.
Competencies
