Under the general direction of the Lead Case Manager, provides case management services to support the implementation of the City’s strategy to reduce homelessness, reduce noncriminal call volume, and reduce escalation of crises. Will assist the Delta Police Department (DPD), Code Enforcement, and Municipal Court by connecting residents with community resources and providing case management services. Primary case management role is street outreach and coordination with PD.

ESSENTIAL DUTIES AND RESPONSIBILITIES

PROGRAM LEADERSHIP & OPERATIONAL SUPPORT
  • Serves as the primary operational support and second-in-command to the Lead Case Manager, assisting with daily coordination of case management services, field operations, and interagency response.
  • Acts on behalf of the Lead Case Manager as delegated, including during absences or emergency situations.
  • Supports implementation of program priorities, service models, and operational protocols.
DIRECT CASE MANAGEMENT & FIELD RESPONSE
  • Provides direct case management services to individuals experiencing homelessness to ensure continuity of care and meaningful connections to services.
  • Works with clients across varying levels of need, including high‑acuity and complex cases.
  • Conducts outreach and provides services in the field; fieldwork is a required and regular function of the position.
  • Assists in the coordination and facilitation of walk‑in case management hours.
  • Maintains detailed, timely, and accurate case notes in case management software systems.
PUBLIC SAFETY & EMERGENCY COORDINATION
  • Works directly with the Delta Police Department and Code Enforcement to meet clients at various service entry points.
  • Responds on‑scene with law enforcement following de‑escalation to provide case management support and service connection when necessary.
  • Monitors and responds to police radio traffic when appropriate.
  • Supports nighttime operations during emergencies, severe weather, and warming/cooling center activations.
  • Supports and helps manage daytime programming during facility activations.
DATA MANAGEMENT, REPORTING & SYSTEMS INTEGRITY
  • Supports the Lead Case Manager for program data integrity, consistency, and accuracy.
  • Oversees data gathering, entry, and reporting across the program, ensuring alignment and consistency between HMIS, internal case management systems, and the Delta Police Department’s Spillman system.
  • Supports data analysis and reporting to inform program operations, service coordination, and performance monitoring.
  • Assists in identifying data gaps, system improvements, and process efficiencies.
PARTNERSHIPS & INTERAGENCY COORDINATION
  • Engages with community partners, service providers, and external organizations to support coordinated service delivery.
  • Supports relationship‑building and ongoing communication with nonprofit, regional, and institutional partners.
  • Collaborates with Shelter Technicians and Facilities staff to ensure a safe, orderly, and supportive service environment.
FACILITY & OPERATIONAL SUPPORT (AS NEEDED)
  • Assists with rule enforcement to maintain safety and order within the facility.
  • Provides limited operational support, including basic cleaning of shared areas, during emergencies or staffing shortages as appropriate.

OTHER
