Schedule:

Monday-Friday 8am-5pm

Summary of

Job Responsibilities

The Team Manager is the heart of the interdisciplinary team ensuring that Hope West delivers high-quality, compassionate hospice and palliative care to patients living in various community settings. From welcoming new team members to fostering growth through regular one-on-one sessions and dynamic group meetings, the Team Manager serves as the driving force for staff excellence. This role is responsible for ensuring that every team member feels empowered to deliver the highest standard of care through a hands‑on leadership approach, including staying connected and addressing any needs promptly.

Serving as a guardian of sustainability, the Team Manager works collaboratively with teams to meet monthly contribution margins while never losing a commitment to quality care.

Qualifications
  • RN with current Colorado licensure; graduate from an accredited school of nursing, BSN preferred.
  • Must have and maintain hospice certification (CHPN) or obtain no later than the end of the second year of employment.
  • At least 3 years of experience in home health, hospital, or hospice setting.
  • Demonstrated management and/or leadership experience.
  • Valid Colorado driver’s license and unlimited access to reliable transportation.
  • Adequate automobile insurance.
Competencies & Requirements
  • Effective writing and speaking skills.
  • Ability to work effectively and professionally with multiple disciplines.
  • Ability to build and maintain rapport and positive working relationships with others.
  • Working knowledge of basic computer skills including use of Microsoft Office products and EMR software.
  • Demonstrates effective problem solving and critical thinking skills.li>
  • Maintains knowledge of organizational, state, and federal policies and regulations pertaining to hospice care.
  • Adaptability and flexibility to changing situations.
Physical Requirements – Medium Work

While performing the duties of this job, the employee is regularly required to stand, walk, sit, use the hands to finger, handle or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. Must be able to lift and/or move up to 50 pounds and push/pull up to 50 pounds.

Specific vision abilities required by the job include close vision, distance vision, color vision, peripheral vision, depth perception and the ability to adjust focus.

Working Conditions
  • Subject to both environmental conditions – activities occur inside and outside.
  • Subject to a variety of physical conditions within individual patient homes: safety issues may include basic home safety, fire safety, environmental safety, and bathroom safety.
  • Subject to occasional exposure to hazards associates with blood and potentially infectious materials.
  • Subject to occasional exposure to hazardous substance and hazardous waste.
Pay & Benefits
  • Exempt/Salaried hiring range: $80,850-$89,440
  • Mileage reimbursement of 65cents/mile for work related driving
