Detailed Overview
Provides physiotherapy services in accordance with the College of Physical Therapists of British Columbia (CPTBC) standards of practice, Fraser Health mission, values and physiotherapy acute care standard, and works with the interprofessional care team to provide patient- and family-centered care. Responsibilities include assessment, diagnosis, prognosis, treatment plan development, implementation and evaluation, and patient education. May provide care through in-person, telephone, virtual, or remote delivery, including outreach and community settings.
Responsibilities
- Provide physiotherapy assessment of the patient’s physical status, functional abilities, needs, and goals; conduct examinations; monitor patient health to improve or maintain function.
- Develop and communicate diagnosis, prognosis, and evidence-based intervention plans with short- and long-term goals.
- Implement interventions per treatment guidelines; educate patients and caregivers on self-management and prevention.
- Participate in interprofessional care planning; update care plans and collaborate on discharge planning.
- Evaluate and document intervention effectiveness; adjust plans to achieve optimal functional outcomes.
- Identify appropriate referrals and coordinate with related services and community programs.
- Collaborate with patients and families, build therapeutic relationships, and respect patient autonomy and privacy.
- Maintain patient records and reports; compile workload statistics and written summaries as requested.
- Delegate components of treatment to rehabilitation assistants, providing direction and ensuring quality care.
- Serve as a resource to colleagues and participate in team activities to promote best practices and safety.
- Contribute to research, special projects, and committee work as assigned.
- Performs other related duties as assigned.
Qualifications
- Current full registration with the College of Physical Therapists of British Columbia (CPTBC).
- Knowledge of physiotherapy theory and practice within a patient- and family-centered model of care.
- Knowledge of CPTBC standards of practice and guidelines for clinical practitioners.
- Knowledge of physiotherapy standardized tests, measures, and equipment; ability to operate related software.
- Ability to communicate effectively with patients, caregivers, and interprofessional teams; ability to teach and consult.
- Ability to work independently and collaboratively; establish workload priorities and adapt to unexpected events.
- Basic knowledge of research processes and methodology.
