Position Summary

As the Patient Care Coordinator, you will act as a patient advocate and support care management services for patients with a single chronic condition or patients with multiple chronic conditions with focus on one high-risk condition. In this role you will help implement individualized care plans, support medication and condition management follow-up, and provide ongoing patient education and coordination based on provider direction.

This role is ideal for someone who is calm on the phone, highly organized, and passionate about helping patients navigate chronic and autoimmune disease care.

What You’ll Do (Key Responsibilities)
  • Serve as a key advocate for patient care by identifying and supporting those eligible for Patient Care Management (PCM) services.
  • Implement patients individualized plan of care who have a single chronic condition or with multiple chronic conditions but focused on a single high-risk condition.
  • Coordinate individualized care plans for patients with chronic conditions, ensuring consistent follow-up and proactive management.
  • Communicate regularly with patients to monitor progress, provide education, and adjust care plans as needed.
  • Support providers in developing and implementing care management strategies.
  • Track all PCM services and document interactions accurately within the EMR.
  • Triage patient phone calls and messages, offering medical guidance per established protocols.
  • Educate patients on autoimmune and chronic disease management.
  • Encourage adherence to treatment plans, follow-up visits, and lifestyle recommendations.
  • Collaborate with physicians, clinical staff, and external partners to ensure comprehensive, coordinated care.
  • Participate in data tracking and outcomes reporting to enhance program quality and effectiveness.
Qualifications
  • 2–3 years of experience in a care management or patient coordination role.
  • Graduate from an accredited nursing or medical assisting program preferred.
  • 3+ years of medical assisting experience required.
  • Rheumatology experience preferred.
  • Bilingual a plus.
