Location: Sussex Campus Hospital

Status: Full Time 72 Hours biweekly

Shift: Days 7a-7p

Salary Range: $39.88 - $61.81 hourly

General

Summary:

The Clinical Coordinator is a member of the management team who assists the Nurse Manager with the daily operations of the nursing unit/department on a 24‑hour basis. The Clinical Coordinator provides direct supervision of staff and rotates weekend, holiday and shifts as needed to ensure management presence and visibility according to departmental needs. They participate in employee selection and evaluation, scheduling and staffing, and in developing and maintaining clinical and budgetary standards.

The Clinical Coordinator leads unit‑based work groups (e.g., PI and unit committees) and coordinates direct patient care.

Responsibilities
  • Demonstrates the ability to collect relevant patient healthcare data. Prioritizes data collection activities as indicated by the patient’s immediate condition, collects data using appropriate assessment techniques and instruments, documents collected data, and uses a systematic and ongoing process.
  • Demonstrates ability to analyze the data in determining patient needs. Assesses data to determine patient needs, interacts with the patient, family and other members of the healthcare team, prioritizes and documents in a manner that facilitates determining expected outcomes and developing a plan of care, and documents patient needs in the clinical record.
  • Demonstrates the ability to identify individualized, expected outcomes for the patient. Outcomes are derived from patient needs, mutually formulated with the patient, family, and other healthcare providers as appropriate, realistic, individualized, culturally appropriate, age‑specific, measurable with a time element when possible, and documented in the clinical record.
  • Demonstrates ability to develop a plan of care that prescribes specific interventions to attain expected outcomes. The plan of care is individualized by the RN to reflect the patient/family needs, developed collaboratively with the healthcare team, reflects current acute care nursing practice, provides continuity of care, establishes priorities, and is part of the clinical record.
  • Demonstrates ability to implement interventions identified in the Plan of Care. Administers patient care interventions in a manner that minimizes complications, encourages the patient and family to participate in the plan of care, and documents interventions in the clinical record.
  • Demonstrates ability to evaluate the patient’s progress toward attaining expected outcomes. Evaluation is systematic, ongoing, criterion‑based, involves appropriate members of the healthcare team, occurs within an appropriate time frame after initiating interventions, uses ongoing assessment data to revise patient needs, revises outcomes and plan of care, evaluates the effectiveness of interventions relative to outcomes, and documents the patient’s response.
  • Demonstrates ability to systematically evaluate the quality and effectiveness of nursing practice. Participates in performance improvement activities and uses their results to initiate changes in nursing practice when appropriate.
