Registered Nurse (RN) Responsibilities:

• Perform home health visits with adult patients throughout lower Sussex County

• Follow a personalized plan of care based on physician orders

• Administer skilled nursing care including wound care, medication administration, and treatments

• Monitor patient status and communicate changes to the clinical team

• Coordinate care with other disciplines (PT, OT, HHAs, etc.)

• Accurately document patient care using an electronic medical record (EMR) system

Qualifications:

• Current RN license in the state of Delaware

• Strong communication and time management skills

• Ability to work independently and travel to patient homes within the assigned territory
