General Summary

The Licensed Practical Nurse (LPN) provides professional nursing care to patients, performing specialized knowledge, judgment, and skills in accordance with Bayhealth policies. After competency verification, the LPN delivers patient care services to the adult and geriatric populations, including vital signs monitoring, medication administration, specimen collection, procedural assistance, and patient education.

Responsibilities
  • The LPN must read and interpret documents such as doctors’ orders, medication names, and lab tests, and perform focal assessments for established inpatients.
  • Provide general basic nursing care, such as positioning patients, lifting and turning, assisting in use of bedpan, commode, urinal or bathroom, and weighing.
  • Provide exceptional customer service that aligns with the Planetree philosophy.
  • Perform routine, uncomplicated treatment procedures including enemas, application of hot and cold compresses, dressing changes, wound cleansing, obtaining vital signs including orthostatic vital signs and rectal temperatures, and splinting of extremities. Collect and record intake and output.
  • Prepare and administer oral medications as ordered by the physician or APC, reviewing medication history with patient.
  • Assist with minor medical/surgical procedures in the practice setting.
  • Perform venipuncture for the purpose of blood collection and for monitoring the patient’s condition before, during, and after withdrawal of blood.
  • Transport patients using wheelchair or stretcher within offices and to/from vehicles; assist those patients requiring help with ambulation.
  • Identify the need for patient education and advise patients on their treatment plan.
  • Demonstrate the ability to recognize abnormal lab results.
  • Document treatment records for each patient, reporting any observed changes in condition or behavior, and any unusual incidents in an acceptable medical‑legal fashion.
  • Set up for suturing and procedures using sterile technique; dismantle procedure set and care for soiled instruments safely and correctly.
  • Maintain competency in specialized testing for Occupational Health and comply with HIPAA and other compliance guidelines.
  • Ensure compliance with Joint Commission regulations, including disposal of used sharps and contaminated materials, and safe storage of medications.
  • Re‑order supplies as required and within budgetary limitations; maintain workstation cleanliness, restock linen and supplies, and perform general cleaning tasks.
  • Engage in an annual self‑assessment, seek constructive feedback, and achieve performance goals.
  • Participate in ongoing educational activities to maintain current knowledge (e.g., BLS, NAPPI) and maintain professional affiliations.
  • Provide support to colleagues and contribute to a learning environment that encourages teamwork.
  • Possibly act as backup for other staff members.
  • Perform other duties as assigned within the scope of the role.
Required Education, Credentials and Experience
  • Education:

    Certificate Program Licensed Practical Nurse or equivalent credit hours in an RN program.
  • Credentials:
    Basic Life Support;
    Licensed Practical Nurse.
  • Experience:

    2 years ED experience as a Tech/LPN, or 2 years LPN with medication administration experience, or a combination thereof.
Preferred Education, Credentials and Experience
  • Education:

    4 years ED experience as a Tech/LPN, or 4 years LPN with medication administration experience.
  • Credentials:
    Certification related to the position if applicable, e.g., Advanced Cardiovascular Life Support.
