Position Overview Impact Life is seeking a full-time Outreach Specialist to support the Home4

Good program, an outreach-focused initiative designed to engage individuals experiencing unsheltered homelessness. This position is part of the Home4

Good program and is responsible for consistent, community-based outreach, engagement, and navigation to housing and supportive services. The Outreach Specialist conducts street-based outreach in nontraditional settings and works closely with the Home4

Good Case Manager to ensure coordinated engagement, warm handoffs, and continuity of care. This role emphasizes outreach, trust-building, and service connection and does not provide long-term or clinical case management.

Responsibilities
  • Conduct consistent, street-based outreach to individuals experiencing unsheltered homelessness in encampments, public spaces, and other community locations
  • Build rapport and trust using trauma-informed, recovery-oriented, and person-centered engagement practices
  • Identify immediate needs related to housing, healthcare, documentation, income, food, and safety
  • Coordinate closely with the Home4

    Good Case Manager to support housing navigation and service connection
  • Provide warm handoffs to internal Impact Life programs and external community partners
  • Accompany individuals to appointments or service connections when appropriate
  • Advocate for participants while maintaining professional boundaries
  • Maintain accurate and timely documentation of outreach contacts, referrals, and outcomes
  • Participate in team meetings, supervision, and program coordination activities
  • Travel throughout the community as part of regular outreach activities
Qualifications
  • High School Diploma or GED required
  • Minimum of one (1) year of experience in outreach, housing services, behavioral health, or a related field preferred
  • Experience working with individuals experiencing homelessness or other vulnerable populations strongly preferred
Certification Requirement
  • Applicants must either possess an active Certified Peer Recovery Specialist (CPRS) credential, or
  • Meet eligibility requirements to obtain CPRS certification and successfully complete certification within one (1) year of hire

Work Environment
  • Primarily field-based, including outdoor and street outreach settings
  • Schedule may include evenings or weekends based on outreach needs
  • Requires travel throughout the state of Delaware
