THE ROLE
Substance Abuse Specialist is a bachelor’s level paraprofessional with at least 40 hours of substance abuse course work, CADC preferred, and clinical experience providing assertive outreach, services linkage, substance abuse counseling, resources, benefits assistance, referral, and housing services for homeless adults with behavioral health problems. The Substance Abuse Specialist is responsible for facilitating program admissions and discharges, for monitoring client eligibility, and for ensuring that appropriate services/service linkages are provided to each person served.
S/he is responsible for case planning, utilization management, data collection, and continual assessment to ensure that persons served are receiving the right level of care to meet their needs, and that services are tracked using the approved management information system.
- Develops relationships with all clients and encourages all clients to become involved in the community and independent as possible while providing necessary substance abuse treatment, counseling, harm reduction, and referral supports.
- Ensures the health, safety, and well-being of all assigned clients is maintained by adhering to internal health and safety policies and PM 46 regulations. Reports any concerns to the supervising Team Lead.
- Must meet visitation and treatment team compliance goals for Community Hospital Coordinated Care as directed by the Team Lead and auspice of the Division of Health and Social Services. Responsible for the completion and submission of CHCC form for every admission of consumer on assigned caseload or as directed.
- Attends daily and weekly treatment team meetings as well as monthly residential house meetings if applicable.
- Ensures that there is a process in place to trigger an immediate increase in service for a person served who experiences a crisis.
- Ensures that persons waiting for services at a higher level receive interim care that addresses their most acute concerns.
- Works in conjunction with the directors of other services internally and externally to ensure that all persons served receive the services they need and achieve a seamless transition from one service level to the next in accordance with their needs.
- Manages the benefits application process and coordination with appropriate State Service Centers, ASSIST, Social Security, Logisti Care, Division of Vocational Rehabilitation, Division of Disabilities Determination, etc.
- Bachelor’s degree in psychology, counseling, human services, social work, nursing, rehabilitation or related field from an accredited college or university.
- 5 years of experience in the field. Mandatory 40 hours or more in 40 hours of education or CEU’s in substance abuse, preferably has a CADC certification.
- Experience working with persons with severe mental illness and/or substance abuse.
- Experience in data collection and evaluation required.
- CPR and First Aid.
- Experience with underserved populations preferred.
- Excellent communication skills and organizational skills.
- Excellent ability to work with clients with substance abuse, co-occurring and/or mental health disorders.
- Excellent writing skills and record-keeping skills.
- Valid driver’s license in good standing with at least 3 years minimum continuous driving experience with no serious traffic infractions.
- Computer knowledge of MS Office, and other HR applications is a plus, not required.
- Possess strong verbal and written communication skills.
- Must be proactive, organized, and possess great follow-up skills.
- Must have the skill/ability to collaborate, partner with, and provide exceptional customer support to a variety of affiliate partners within the organization.
- Three- year Motor Vehicle Record and valid Delaware driver’s license.
