Job DescriptionThe Team Lead Case Therapist II reflects the mission, vision, and values of NM, adheres to the Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines, and accreditation standards. This role is responsible for providing intensive case management services to individuals with chronic and/or persistent mental illness or co‑occurring disorders.
Responsibilities- Assist clients in engaging in needed services and finding community resources.
- Help clients apply for and re‑determine benefits assistance.
- Support clients in obtaining basic necessities such as food, housing, and safety resources.
The approach is team‑based: the team shares the overall caseload, but each member is individually assigned specific clients for consistent follow‑up and support.
PopulationClients have higher needs and are at higher risk of crisis and instability than those engaged in traditional case management services.
Schedule40 hours/week, Monday‑Friday, 8:30–5:00. No overnight or weekend hours.
Qualifications Required- Experience and training in assessment, individual, family, and group counseling.
- Master’s Degree with LSW, LCSW, LPC, or LCPC required, or eligible for licensure.
- Master’s degree in social work, psychology, counseling, or a related field.
- 3‑5 years in an intensive treatment and/or mental health setting.
- CADC licensure.
- LIC CPC.
- LIC LCSW.
- LIC MSW.
Northwestern Medicine conducts a background check that includes criminal history on newly hired team members and, at times, internal transfers. Sign‑on Bonus EligibilityInternal employees and rehires who left Northwestern Medicine within 1 year are not eligible for the sign‑on bonus. Exception: new graduate internal employees seeking their first licensed clinical position at NM may be eligible, depending on job family.
