Social Worker - Disabled Children's Service (11+ Team)Location: Rhondda Cynon Taf County Borough CouncilSalary: £43.60 UMBRELLA per hour (£33 PAYE)Contract: full time, 27 hours per week, Initially 3 months
We are working on behalf of Rhondda Cynon Taf county Borough Council and an exciting opportunity has arisen to join their Disabled Children's Service, supporting children and young people with lifelong, permanent and substantial disabilities.
The service is made up of five specialist teams:
- Enablement Team
- 0-11 Team
- 11+ Team
- Eligibility Team
- Occupational Therapy Team
About the 11+ Team The 11+ Team provides dedicated care and support to children and young people with:
- Complex physical disabilities
- Learning disabilities
- Life-limiting conditions
- Promoting independence
- Managing complex and evolving needs
- Preparing young people and their families for adulthood
- Developing tailored, outcome-focused support plans
Key Responsibilities
- Undertake comprehensive assessments to identify needs and outcomes.
- Develop, implement and review tailored care and support plans.
- Support young people and families through transition planning processes.
- Work closely and collaboratively with Adult Services to ensure seamless transitions.
- Promote independence and person-centred planning for adulthood.
- Undertake Mental Capacity Assessments where appropriate.
- Complete relevant administrative processes to support applications to the Court of Protection where required.
- Work within a multi-agency framework to coordinate complex packages of support.
- Maintain high-quality written records, reports and statutory documentation.
- Has experience working with disabled children and young people with complex needs.
- Understands transition planning and the challenges young people face moving into adulthood.
- Has knowledge of Mental Capacity legislation and Court of Protection processes (desirable).
- Is confident working within a multi-agency environment.
- Demonstrates strong assessment, planning and report-writing skills.
- Is passionate about promoting independence and achieving positive outcomes.
- Be part of a specialist, supportive multi-disciplinary service.
