Job Overview

Reliance Home Health Caregivers is seeking a licensed nurse to provide long‑term private‑duty care for a 5‑year‑old male patient in Deerfield, IL 60015. The position requires coverage every Monday, Tuesday, and Thursday from 7 pm to 7 am.

• Provide direct patient care and monitoring during assigned shifts.
• Administer medications and treatments, manage tube feeds, trach care, and respiratory equipment.
• Collaborate with case managers, schedulers, and other team members to ensure continuity of care.
• Document patient status, interventions, and care plans accurately and timely.

• Valid LPN or RN license in the state of Illinois.
• Minimum 1 year of direct patient care experience (RN/LPN). Pediatric experience preferred but not required.
• Current CPR certification.
• Ability to work flexible hours and multiple shift lengths (8, 10, 12, 16 hours).
• Excellent communication and teamwork skills.

Hourly: $38 for LPN / $47 for RN.

• Flexible scheduling for full‑time, part‑time, or PRN.
• Supportive schedulers and case managers.
• Training available in pediatrics, home health, and additional skills.
