- Verify the Plan of Care before each visit and provide care according to delegation by RN, physician orders, assessment data, and established standards and guidelines.
- Incorporate patient care goals into nursing visits, evidenced by nursing note documentation.
- Communicate significant findings, problems, and changes in condition or environment to RN Case Manager, physician, clinical supervisor, and other personnel.
- Report unsafe conditions to the appropriate RN Case Manager, clinical supervisor, and physician.
- Implement the plan for patient safety using available resources.
- Assist patients in learning self‑care techniques while delivering quality care.
- Suggest revisions to the Plan of Care in response to issues and notify RN Case Manager.
- Provide services that require substantial and specialized nursing skills under direction of RN or physician.
- Collaborate with RN Case Manager for advice, assistance, and instructions.
- Document medication regimens and update medication profiles per procedures.
- Treat patients and caregivers with the highest care.
- Implement actions to prevent avoidable hospitalizations and ER visits.
- Provide positive, supportive communication to physicians, patients, families, visitors, and agency personnel.
- Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
- Current State License as a Licensed Vocational / Practical Nurse.
- Minimum 1 year of experience in a clinical care setting.
- Experience in hospice or home health preferred.
- 50% travel required.
- Capacity to sit, stand, bend, lift, and move intermittently; able to lift 50‑100 lbs.
- Valid driver’s license, insurance, and reliable transportation for job duties.
Clinical Team Manager.
Salary$27.00 to $32.00 per hour, based on education, experience, skills, and internal equity.
