Overview

RN Case Manager

Comprehensive clinical evaluation, assessment and documented care plan in all domains including social determinants of health, psycho-social, financial, medications, ADL/IADLs and cognitive, coordinating and monitoring all services needed with a focus on providing education and the goal of self-management skills.

GA license;
Minimum five (5) years of RN Clinical Nursing, social work, case management or utilization management (UM).

• Equivalent combination of case management/social work/UM training

Length of assignment: 13 weeks

Shift / Hours per Week: M-F 8:00-5:00;
Every other weekend; 40
