Visiting Nurse Health System is seeking an RN Case Manager for Hospice Atlanta who provides nursing care, emotional support, and education to terminally ill patients and their families. The ideal candidate demonstrates clinical expertise in nursing assessment and diagnosis, and advanced proficiency in the technical skills necessary to develop, provide, and coordinate patient care. Our Hospice Clinical team is dedicated, committed, and passionate, and we desire a candidate who believes this work is a calling on their life.
This role is headquartered at our Marietta office and covers the DeKalb and Decatur territory.

In this position…
* Assesses patients for admission to the Hospice Atlanta Program. Admits patients for care based on skilled need and payor source eligibility requirements.
* Assesses & coordinates the need for additional services to enhance patient care.
* Coordinates care between home, outpatient, and inpatient settings. Completes transfer form and initiates inpatient log for medical patients.
* Makes referrals to other agencies on the patient's behalf.
* Evaluates the patient's response to care as evidenced by the visit note.
* Provides education to the patient & family regarding the disease process, self-care techniques, and prevention strategies, as evidenced by visit note documentation.
* All Clinical documentation is complete and accurate and submitted to the office per policy and guidelines.
* Participates in on-call rotation per program requirements.
* Coordinates patient/family care provided by interdisciplinary team members.
* Clinical record documentation demonstrates compliance with physician orders for care provided and frequency of visits made.
* Obtains physician s orders for care and services provided per policy.
* Documentation demonstrates compliance with Medicare, Medicaid, and other regulatory requirements.
* Demonstrates knowledge of infection control standards and techniques as per VNHS policy and procedures and consistently practices these techniques.
Do you have…
* Associate Degree or Baccalaureate School of Nursing.
* 1 year of professional nursing experience.
* Must have a current licensure by the Georgia State Board of Nursing and a valid CPR certification.
* Knowledge of Medicare, Medicaid, and other funding source regulations as applied to hospice care services.
* Valid GA driver's license, proof of auto insurance, and reliable transportation

Compensation: $72,248 - $108,372
