Allegria Village, a senior living community in Dearborn, is seeking a Full
-Time MDS Coordinator who is a Registered Nurse (RN) to join our Skilled Nursing Facility team.

This position is strictly for a Licensed Registered Nurse (RN). LPNs will not be considered.

Position Summary

The MDS Coordinator (RN) is responsible for the accurate and timely completion of the Minimum Data Set (MDS) and the Resident Assessment Instrument (RAI) process in compliance with all federal and state regulations. This role plays a critical part in clinical accuracy, regulatory compliance, quality outcomes, and reimbursement for skilled residents.

Ensure accurate and timely completion of all MDS assessments in accordance with the RAI Manual and federal/state regulations.

Review all admissions for completeness of electronic medical records and open required assessments per RAI guidelines.

Prepare and manage assessment schedules; notify interdisciplinary team members accordingly.

Conduct and document resident interviews as required by regulatory agencies.

Ensure all resident diagnoses are accurate, complete, and properly coded for billing purposes, including therapy-related diagnoses; collaborate with physicians as needed for clarification and documentation.

Assist with case management for skilled residents, including Medicare Part A and managed care.

Maintain and update all hard copy and electronic tracking tools, spreadsheets, and skilled documentation per company policy.

Coordinate with nursing, therapy, social services, admissions, discharge planners, and leadership to support care planning and positive discharge outcomes.

Prepare, submit, and track electronic documentation for managed care authorizations and federal/state agencies, including MDS submissions and Michigan LOCD.

Maintain Final Validation Reports (hard copy or electronic) for a minimum of seven (7) years.

Support facility audits, claims reviews, ADRs, and state/federal surveys, including CMS 671/802 updates.

Generate and distribute CMS reports from CASPER and iQIES to leadership, including:

  • MDS-related reports
  • SNF Quality Measure reports
  • SNF QRP Review and Correct reports
  • Value-Based Purchasing (VBP) Confidential Feedback reports

Provide education and clarification to nursing staff and interdisciplinary team members related to MDS data collection and completion.

Perform other duties as assigned.

Qualifications

Must be a Licensed Registered Nurse (RN) — LPNs are not eligible for this position.

Minimum 1 year of MDS experience in a Skilled Nursing Facility required.

Demonstrated understanding of MDS coding and its impact on federal and state reimbursement programs.

Strong knowledge of federal and state regulations, RAI Manual requirements, and compliance standards.

Excellent organizational, communication, and interdisciplinary collaboration skills.

Education & Certifications

Graduate of an accredited Registered Nursing program.

Active RN license in the state of Michigan.

Current CPR, First Aid, and AED certifications.

Physical Requirements

Ability to lift a minimum of 50 lbs.

Ability to walk, stand, bend, stoop, stretch, push, and type throughout the workday.

May be exposed to residents with communicable diseases.
