As a PT in this role, you will become an integral teammate in our innovative Take Your Therapist Home Program!

Join us at Rivetus Rehab! This role is focused on Home Health Care in the area surrounding Dearborn Heights and Allen Park with a steady referral source from multiple skilled nursing facilities. This role includes PTA supervision and provides the opportunity to pick up additional time at a nearby SNF.

What is Take Your Therapist Home (TYTH)? In a world of shorter and shorter SNF stays, TYTH is a unique program where clinicians work directly with patients in the SNF and home health, providing a continuity of care that allows you to have a greater impact on patient outcomes.

Dearborn Heights and Allen Park. Our Dearborn Heights facility is located at 26001 Ford Road Dearborn Heights, MI 48127 and our Allen Park facility is located at 9150 Allen Road Allen Park, MI 48101. This is a home care role providing Home Health therapy services with the option to have additional time in the SNF. Home Health geographic area is the vicinity around Optalis Health & Rehabilitation of Dearborn and Allen Park.

Monday – Friday, Full Time

• Successful completion of an accredited Physical Therapist program
• Licensure as required by the State of Michigan

• Ability to exercise professional judgement and initiative when analyzing problems and recommending solutions
• Initiative and flexibility in the work environment
• Ability to react effectively to the needs of patients and their families
• Physical ability to bend, position, transfer, and lift patients weighing from 80 pounds to over 300 pounds without restriction, utilizing lifting devices clinically recommended for the patient
