Base pay range

$80,000.00/yr - $90,000.00/yr

PREVENTION
  • Educates staff on the prevention of pressure ulcers and protection of skin integrity
  • Makes rounds on off shifts periodically to ensure preventive measures are in place – i.e., padding between bony prominences; re‑positioning; use of barrier creams and/or ordered protective devices (specialty mattresses, floating of heels, pressure reduction in chairs; etc.); discusses findings with ADON of unit, nurses and CNAs as appropriate.
  • Completes Braden Scale as required by Federal regulation – admission / weekly x 4 weeks after admission; monthly x 3 months, then quarterly thereafter.
  • Follows up with RD as to evaluation of nutritional needs; ensures that RD recommendations are followed. Discusses lab results with RD as needed.
  • Discusses with pharmacy consultant and resident’s physician, the use of skin‑enhancing vitamin supplements, such as Protegra.
TREATMENT
  • Performs treatments on Stage III and IV wounds
  • Assigns Stage I and II to floor nursing staff, oversees completion of all treatments
  • Monitors weekend treatment effectiveness; contacts the facility on weekends if special attention is needed for specific wounds, to ensure continuity of care.
  • Contacts physician if treatment is not effective or if for some reason resident/patient is not responding to treatment.
  • Discusses possibility of PT assistance in treatment of specific wounds, through interdisciplinary team meetings or referral systems.
DOCUMENTATION
  • Ensures that weekly skin assessments by licensed staff are performed; does periodic look‑behind assessments to confirm accuracy of documentation;
  • Monitors TARs for treatments of wound conditions that are not personally treated;
  • Documents all treatments provided with records of MD consults, changes in MD orders, and progress reports. Measures and stages all Stage III and IV wounds weekly.
  • Oversees documentation of all preventive efforts – supplements, RD recommendations / follow up; restorative intervention if applicable, turning and repositioning.
  • Documents weekly wound assessment record.
  • Completes the MDS 3.0 Section M accrording to the MDS 3.0 schedule, the CAAS as appropriate, and participates in care planning based upon the CAAS.
CARE PLANNING
  • Wound Manager updates care plans on a routine / regular basis as resident progresses (or not) with new interventions as required. Ensures that care plan is completely current at all times.
  • Educates CNAs and licensed staff on content of the care plan as it relates to approaches impacting them.
  • Keeps DON advised as to progress.
  • Communicates with families at least weekly or bi‑weekly to inform them of progress of their relative concerning any wound present. Documents communication.
EVALUATION
  • The Wound Manager is continually evaluating progress in all areas, including prevention programs, turning and repositioning consistency, RD involvement and nutritional intervention, MD contact, resident involvement in care, care planning interventions and progress, and any other action taken to prevent or heal wounds.
  • The Wound Manager is a part of the interdisciplinary team, and must communicate at all levels effectively, with the DON, ADON’s on the units, therapists, activities, and social services. Participates in plan of care meetings with families as needed. (See note above re consistent family communication).

If the Wound Manager has difficulties with any level of staff regarding wound care, prevention or attention, s/he will discuss openly with DON with a plan to correct issues as identified.
