Licensed Practical Nurse – Vital Core Health Strategies
Pay RangeBase: $25.00/hr – $34.00/hr (actual pay based on skills and experience)
Position SummaryThe Licensed Practical Nurse (LPN) delivers quality care consistent with the scope of practice outlined by the local state nurse practice act. The LPN is responsible for tasks, activities, and functions delegated by the HSA, DON, RN, or Medical Director and may assign duties to others as defined by the state act. The LPN monitors patients as directed and reports changes to appropriate medical staff.

- Full Time
- Night Shift
- Graduate from a Licensed Practical Nursing program.
- Currently licensed as a Practical Nurse in the state of employment.
- Possesses an active CPR certification.
- Remains knowledgeable about state laws and regulations governing practice.
- Completes initial and annual clinical competencies as assigned by role.
- Utilize a systematic approach to meet the health needs of each patient.
- Implement nursing care within the LPN’s scope of practice, complying with all applicable laws.
- Assist in developing and implementing teaching plans based on patient needs.
- Provide wound care with cleaning and dressing/bandaging.
- Administer medications to offenders.
- Care for multiple patients as directed by the Medical Director, DON, or Registered Nurse.
- Monitor vital signs and report changes to medical staff.
- Document actions in the MAR and medical records.
- Perform other nursing duties as assigned by the facility.
