• Effectively represents the organization and the clinical perspective to governing bodies and external audiences, ensuring a consistent, principle‑driven brand for clinical practice across the system.
• Leads the execution of the strategic vision for care management across the organization, enhancing the consumer experience and deploying Care Management resources effectively.
• Develops strategies to reduce length of stay, readmissions, and clinical denials while simplifying daily workflows and supporting improved governance and accountability structures.
• Implements policies to decrease variability in care management practices and embed evidence‑based care transition practices.
• Advises and supports the standardization of care management roles, responsibilities, job descriptions, and accountability metrics across the enterprise.

• Master's [Required]

• Master's in Social Work OR Bachelor's in Social Work & Master's in related field

• 5+ years of Care Management experience [Required]
• 3+ years of Care Management Leadership experience [Required]
• 2+ years of social work experience [Required]
