We are growing and seeking a Primary Care Physician to play a key role in our value-based care model—partnering with specialists to manage chronic disease, coordinate care, and improve long-term patient outcomes.

With over 130 podiatric medical and surgical clinics in 8 states, Upperline Health providers coordinate patients’ care among a team of specialists including podiatric physicians and surgeons, Primary Care Physicians, Advanced Practice Providers, Care Navigators, Pharmacists, Dietitians, and Social Workers for integrated treatment that addresses patients’ immediate and long-term health needs. We put patients at the center of value-based care.

Upperline Health is seeking a well-trained and compassionate Primary Care Physician (MD/DO) to join our growing care team. This position provides a unique opportunity to practice within a multidisciplinary care model designed to proactively manage primary care needs along with Chronic Disease Management. This position is clinic-based with the ability to provide both consultative and longitudinal care, supported by care coordination teams and advanced technologies.

• Deliver comprehensive primary care services, including preventive care, acute illness treatment, and management of chronic conditions.
• Conduct patient assessments through medical history, physical exams, and diagnostic evaluation.
• Diagnose conditions, develop treatment plans, and prescribe medications and therapies as appropriate.
• Order, review, and interpret laboratory tests and diagnostic imaging.
• Coordinate and manage referrals to specialists and follow up on patient outcomes.
• Educate patients and families on diagnoses, treatment options, medications, and preventive health strategies.
• Maintain timely, accurate clinical documentation in the electronic health record.
• Collaborate with nurses, medical assistants, and other healthcare professionals to ensure continuity of care.
• Comply with clinical policies, quality standards, and regulatory requirements.
