RN Wound Care Coordinator – Full Time, Days
Kindred at Scion Health Join our team to lead and manage the facility wound care program, providing direct patient care, consultative services, and education to nursing staff.
Job SummaryManages the wound care program, establishing standards of care, creating policies, and providing direct patient care and staff support. Acts as the primary consultant for wound care clinicians and collaborates with physicians and other healthcare agencies to optimize wound management and prevention.
Essential Functions- Develops and implements the facility wound care program in conjunction with national standards, creating protocols, documentation tools, formulary, and referral criteria.
- Establishes standards of care, competencies, and policies that improve quality, reduce costs, and enhance effectiveness for all clinical staff.
- Acts as primary consulting resource to wound care clinicians and facility staff, advocating with physicians and enabling effective wound assessment and care plans.
- Provides ongoing education to staff on wound care products and best practices.
- Evaluates all wounds upon admission and throughout care to determine treatment plans and identify early problems.
- Provides consultation and/or assessment for patients with pressure injuries and wounds that do not show measurable healing within two weeks.
- Collaborates with physicians to develop comprehensive wound care plans and prevention strategies.
- Recommends changes to wound care orders, providing evidence‑based research support as needed.
- Reviews patient records to ensure accurate documentation of wound assessments, care plans, and treatments.
- Investigates adverse wound events through root cause analysis and develops action plans with nursing leadership.
- Participates in clinical outcome monitoring, follow‑up, and agency performance improvement initiatives.
- Contributes to CMS quality data reporting via wound‑related LTRAX data sets.
- Collaborates with Clinical Operations to maintain a cost‑effective wound care formulary.
- Assists with training new hire Wound Care Coordinators within the Hospital Division.
- Effective communication and interpersonal skills to maintain working relationships with all hospital departments and to educate patients and families.
- Adaptability to new situations, prioritization, and problem‑solving techniques.
- Knowledge of wound care consistent with NPIAP, AHCPR, WOCN, and CDC guidelines.
- Proficiency in wound debridement within the scope of practice.
- Ability to serve as a resource to nursing staff for complex wound management.
- Leadership skills to motivate, develop, and collaborate with a team.
- Program management and organizational skills.
- Fluent English speaking, reading, writing, and consistency in attendance.
- Travel requirement: 0%.
- Performs related duties as assigned.
- Degree from an accredited nursing program.
- Current state RN license; BSN preferred.
- Professional certification WOCN, CWS, or WCC, or to obtain within 12 months of employment.
- BLS required.
- 1–3 years of licensed professional nursing experience, preferably in acute care.
Full‑time, Days (5 days a week).
