Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Sanctuary Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Monday - Friday, On-Call Weekday & Holiday Rotation

Darke, Preble, Montgomery, Champaign, Clark, Greene and Miami.

• Respond to clinical referral information with care and expertise, addressing inquiries and requests.
• Partner closely with the Clinical Director to assess eligibility and suitability of clients for our specialized hospice services.
• Provide comfort and support for patients, ensuring their comfort and quality of life remain paramount.
• Deliver top-notch nursing care rooted in proven principles and techniques.
• Empower patients and families through education, encouraging active participation in personalized care plans.
• Share call duties to provide support when it matters most.

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice experience a plus.
• Computer proficiency in typing and various programs, including background in EMR.
• Current CPR certification if required by state.
• Current driver’s license, vehicle insurance, and dependable transportation.

$80,000 - $92,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
