Position: RN Case Manager - Home Health Agency (Full Time) | Dayton, OH - SE OH

RN Case Manager – Home Health Agency (Part Time) | Dayton, OH – SE OH

Home Care Network (HCN) is seeking a highly skilled, experienced RN Case Manager to join our home health agency and serve existing and new clients across a 1.5 hour radius in Southwest Ohio
, including Dayton & Dayton Metro, Middletown, Franklin, Troy, Piqua, Springfield, parts of West / Southwest Columbus Metro, and surrounding areas. This is a long‑term employment opportunity that can be built to fit your schedule – whether that’s:

  • as a 2nd job (working around your primary schedule)
  • as a consistent Part‑Time position
  • starting at Part‑Time and increasing to Full‑Time
  • or starting at Full‑Time (requires occasional travel 2+ hours from home)

More about the role:

  • Typical visits take place during a standard 8 am – 5 pm weekday schedule, but many clients attend school and cannot be seen until 3–5 pm, so occasional after‑hours visits may be required.
  • As a Part‑Time position – pay is hourly, paid every week. Typical travel distances include a 40–90‑minute radius.
  • As a Full‑Time position – pay can be hourly or salary, also paid every week. Typical travel distances include a 60–90‑minute radius daily, with up to or beyond 2 hours on occasion.
  • Services include starts of care (SOC), recertifications, resumptions of care (ROC), and other assessments and services, with occasional brief visits as needed.
  • Door‑to‑Door mileage reimbursement is provided.
  • At least 1 year of experience in case management is required.
  • 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.

Compensation:

  • Part‑time & Full‑time hourly: starting at $35/hr
  • Full‑time annual salary (+ PTO): starting at $72,800

Job Summary

As an RN Case Manager, you will coordinate and provide patient care and ensure optimal outcomes. With core skills in managed care, patient assessment, client care, treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care. Your experience in critical care, infectious disease care, and supervising home and community care will enhance your ability to lead and support a multidisciplinary team.

Reports to:

Regional Administrator, or Clinical Administrator/Director of Nursing

Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:

  • Perform client assessments, directing, planning, and initiating appropriate actions in home‑care situations.
  • Manage/supervise a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • Document, communicate, and coordinate patient status/changes in condition to the care team.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one year of nursing supervisory experience; home‑care experience preferred.

Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN license and ability to practice in the State of Ohio.
  • CPR and First Aid certifications required (or obtainable prior to hire date).
  • 1 year of case‑management experience required; home‑health experience highly preferred.
  • Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
  • Strong background in nursing, patient care, and confidentiality.
  • Strong attendance and dependability record with previous employers.
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail‑oriented, accountable, friendly, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Current driver’s license and reliable transportation.
