Suncrest Hospice is looking for a dedicated Hospice Registered Nurse (RN) to provide exceptional care and support to patients and their families.

• Provide care and services for a group of hospice patients at private homes, skilled nursing facilities, and assisted living facilities.
• Collaborate daily with nurses, social workers, chaplains, volunteers, and aides as part of an interdisciplinary team.
• Conduct psych‑social assessments, interventions, and educate patients and families on community resources.
• Manage visits, documentation, and meetings, prioritizing urgent needs and triaging multiple concerns.
• Prepare and maintain accurate patient records in electronic medical records system.
• Travel within the service area, requiring reliable transportation, valid driver’s license, and auto insurance.

• Current Registered Nurse (RN) licensure in Ohio, in good standing.
• Excellent time‑management and communication skills.
• Ability to work independently and in a team environment.
• Familiarity with hospice rules and regulations.
• Experience with electronic medical records is preferred.
