Licensed Practical Nurse (LPN) - DaVita Skilled Nursing Dialysis (DSD) – The Laurels of Huber Heights
DaVita is seeking an LPN to support dialysis care in skilled nursing facilities (SNF) as part of the DaVita Skilled Nursing Dialysis Services (DSD). Role involves delivering compassionate care to patients with end-stage renal failure or chronic kidney disease, in collaboration with dialysis RNs. This position is 3-4 days per week, 12+ hour shifts, with potential travel to cover other SNF clinics in the region and alternating Saturdays.
Training provided; no on-call requirements.

Note:

- Provide direct one-on-one patient care to ensure safe, comfortable, and hygienic dialysis treatment.
- Work in conjunction with dialysis RNs to deliver dialysis care.
- Meet patient needs under a start-up style program, collaborating with cross-functional resources; have autonomy and ability to travel between SNFs as required.
- Participate in training programs, including classroom and hands-on learning, and support other local SNF clinics as needed.
- Deliver care to patients with multiple comorbidities and develop treatment plans leveraging a broad nursing skill set.
- Adhere to CMS guidelines and complete required training prior to operating equipment (e.g., water treatment system) as required by state regulations.
- Current LPN/LVN license in the practicing state.
- Current CPR certification.
- IV certification if required by state regulations.
- High school diploma or equivalent.
- Minimum of one year nursing experience in hospital or clinical setting preferred.
- Completed training program approved by the medical director and CMS guidelines prior to operating related equipment, as required.
- Basic computer skills; proficiency in DaVita systems within 90 days of hire.
- Successful completion of pre-employment color blindness test (accommodations may be provided).
