CARE MANAGER – RN

Premier Health Partners is seeking a dedicated RN Care Manager to deliver systematic, patient‑centered care coordination within our Physician Practice. The RN Care Manager will coordinate with care team members and settings, develop individualized care plans, and monitor high‑risk registries to promote quality outcomes.

• Design and document a mutually agreed plan of care based on patient goals and health risks.
• Perform ongoing assessment, monitoring, and tracking in the electronic health record; use registry functionality to trigger interventions.
• Provide proactive care and education outside routine office visits via phone, Patient Portal, or other access activities.
• Build a trusting, professional relationship with patients, families, and caregivers.
• Utilize self‑management and shared decision‑making tools.

• BSN required.
• Active RN licensure issued by the State of Ohio Board of Nursing.
• Minimum three years of nursing experience; leadership experience preferred.
• Solid knowledge of chronic conditions and strong assessment and interview skills.
• Demonstrated accurate and timely data entry and computer skills.
• Excellent written and verbal communication skills.
• Self‑directed, independent work style with ability to prioritize and multitask.
• EPIC knowledge preferred.
