JOB TITLE:

RN Case Manager

This PRN role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), with occasional new hire orientations and supervisory reviews, among other RNCM assessments and services. On occasion, Skilled Nursing visits may be requested but are optional.

Overview

Service area:
South West Ohio, primarily Dayton + Dayton Metro Area. Qualified applicants may reside in or near any of the service areas so long as travel requirements are fulfilled to all stated communities.

Responsibilities
  • Coordinate and provide patient care with core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning.
  • Oversee and/or supervise a multidisciplinary team in home health settings; document/communicate patient status to the care team.
  • Travel within stated counties to provide services; ensure safe and compliant practices.
  • Ensure effective communication with physicians, agency staff, service administrators, other caregivers, clinicians, and the client and family.
  • Perform job duties while promoting personal safety and a safe environment for clients and coworkers.
Qualifications
  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN license to practice in the State of Ohio.
  • CPR and First Aid certifications required (or can be obtained prior to hire date through the agency or accredited third parties).
  • 1 year experience in case management and home health required; 2+ years of home health experience highly preferred.
  • Experience in supervisory roles, wound care, lab draws, and OASIS assessments is a plus.
  • Willingness to travel throughout all stated counties in the service area.
  • Familiarity with HIPAA regulations and medical terminology; current driver’s license and reliable transportation.

  • At least 1 year of Case Management experience and home health agency (or equivalent) experience is required.
  • Must have experience with OASIS and NON-OASIS assessments (OASIS experience a plus).
  • 1+ years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
Schedule &

Employment Type
  • Employment Type:

    PRN with potential to increase to consistent Part-time if desired (not required).
  • Flexible and customizable schedule; typical hours Monday–Friday (8am–5pm).
  • No On-Call requirements;
    No Overnight shifts required. Potential to expand to Part-Time or Full-Time for the right candidate.
  • Weekend, Overtime, Respite & PRN services are offered when available but not required. PRN/Part-Time RNCMs are paid hourly (weekly on Fridays).
Travel & Service Area
  • Primary service area is focused in Dayton + Dayton Metro Area; coverage across a 90-minute region of surrounding communities is required.
  • Travel distances typically 30–60 minutes daily radius; 1–2 hours max on occasion.
Compensation
  • PRN Hourly Wage: $35 - $40/hour.

Note:

This is a PRN / As Needed position with hours and shifts varying week to week. Candidates seeking long-term employment are preferred, but short-term or temporary employment will be considered.

Reports to:

Regional or Clinical Administrator / Director of Nursing.
