Gracehaven is seeking a compassionate and dedicated Community-Based Case Manager & Clinician to join our team. You will provide trauma-informed clinical services, case management, and support to youth, families, and the broader treatment team.
Responsibilities- Individual and family therapy
- Crisis intervention and safety planning
- Comprehensive assessments and treatment planning
- Collaboration with families, community partners, and other providers
- Client advocacy and discharge planning
- Maintaining accurate documentation and meeting quality assurance standards
- Master's degree in Social Work/Counseling or a related field
- LSW or LPC license in Ohio (or eligible)
- Experience working with trauma, especially sexual violence and trafficking
- Excellent clinical and communication skills
- Agreement with the Central Ohio Youth for Christ and Gracehaven Standards of Conduct and Statement of Faith
