This will be a Full-Time day shift position
Pay Range: $23.25 - 27.25 / hour based on experience
Position SummaryResponsible for the follow-up, completeness, deadline-driven compliance, time sensitive documentation flow, and selected operational monitoring across the Home Care program. This role ensures documentation, transitions, and critical operational support are completed accurately and on time to protect compliance, billing, census integrity and care delivery.
Position QualificationsMinimum Qualifications: License Practical Nurse from accredited school of nursing with a current Ohio license preferred or medical office experience required. Valid driver’s license with a safe driving record. Proof of auto insurance.
Minimum Experience: One year of home health care experience or healthcare operations. Strong attention to detail, follow-up and deadline management skills.
Job Duties- Is responsible for Compliance and documentation oversight to include, but not limited to collect and track all face to face (F2F) documentation, while monitoring deadlines and escalate risks.
- Monitor and resolve Electronic Visit Verification (EVV) exceptions timely.
- Complete and coordinate McBee documentation corrections timely.
- Maintain organized, audit ready compliance trackers.
- Communicate with the clinician and document within the EMR new faxed orders and lab results, faxing lab results on to appropriate physician.
- Complete time-sensitive data entry from portal to portal to include, therapy documentation and other clinical documents impacting compliance, billing or care delivery. Ensure documents are entered promptly, indexed to the correct patient & episode and filed accurately and timely. Monitor flow to prevent delays in compliance or billing and flag/escalate unclear, incomplete, or missing documentation.
- Will provide transitions and census management to include, tracking hospital and SNF patients daily; monitor admissions, discharges, transfers and communicate and document accordingly; monitor discharge status line daily as well verify discharge completion and accuracy; reconcile daily census and identify gaps and prepare transition and census summaries as required.
- Monitor intake flow oversight while identifying stalled, incomplete or high risk referrals. Prompt staff to resolve gaps and ensure effective handoff’s within the process are completed timely, while escalating unresolved issues to leadership.
- Serve as first line support for Netsmart workflow issues, while providing data cleanup and corrections as identified. Escalate reoccurring system and workflow issues to leadership.
- Maintain supplies & inventory weekly, identifying trends and monitoring orders.
- As needed, serve as the primary liaison between field clinicians and contracted Durable Medical Equipment (DME) vendors, track requests, coordinate communication, monitor fulfillment and documentation.
- Adhere to all policies and procedures of the organization and all federal and state laws
- Performs other duties as directed.
Frequent bending is required, walking, sitting, and standing is involved 33% or more of the time. Heavy lifting is required up to 50 lbs. of force occasionally and/or 20 lbs. frequently, and/or up to 10 lbs. constantly to move objects. Potential for exposure to heavy weight bearing > 150 lbs. alone in sudden emergency situations.
