Physical Therapist Opening - with SIGN ON BONUS

• Evaluate patients for rehabilitation services within 24 to 48 hours of physician’s orders.
• Develop effective treatment plans and treat patients according to the plan of care.
• Supervise the Licensed Physical Therapy Assistant in the delivery of clinical services and interactions with other staff, patients and family members.
• Communicate with physicians, clinical managers, and other health team members, patients, families and facility staff regarding patient progress, problems and plans.
• Participate in Plan of Care Meetings and Interdisciplinary Team Meetings as requested.
• Achieve or exceed the minimum productivity standard established by Concept Rehab, Inc. (CRI).
• Participate in in-service training programs for other staff in the facility.
• Record treatment notes, progress notes, provide objective input for discharge summaries, and complete daily billing of services per policy and procedure.
• Instruct patients’ families and/or facility staff in maintenance programs and participate in discharge planning.
• Recommend any needed equipment to facilitate improved delivery of clinical service; report equipment problems to facility personnel.
• Act as a clinical supervisor for affiliating Physical Therapy and Physical Therapy Assistant programs for area universities and colleges.
• Occasionally work beyond usual hours for facility emergencies, weekends and holidays.
• Maintain the mission, vision, core values and professional image of Concept Rehab, exercising good judgment in performance of the job.
• Perform special projects and other duties as assigned.

• B.S., M.S., Masters or DPT in Physical Therapy.
• Licensed in the state(s) where services are being provided as a Physical Therapist.
