Join Summit Home Care and Hospice as a Physical Therapist (PT) to supporting and treat patients in Assisted Living (AL) and Independent Living (IL) facilities, in addition to in-home visits

You’ll enjoy a consistent caseload, reduced travel time between visits, and the chance to build strong relationships with patients and facility staff.

Physical Therapist (PT) Details:

• Location: Dayton, OH & Surround Areas
• Schedule: PRN | Flexible Scheduling
• Pay per Visit: $60.00 - $120.00 (Based on Visit Type)
• Travel Required: Yes - Mileage Reimbursed at $0.48/Mile

What You'll Do:

• As a PT, you’ll play a vital role in helping patients regain mobility and independence
• Delivering physical therapy services per physician’s care plan and state regulations
• Assessing patient mobility, strength, and rehabilitation potential
• Designing and implementing personalized therapy programs
• Educating patients and caregivers on therapeutic techniques and equipment
• Collaborating with interdisciplinary teams to ensure holistic care
• Supervising PTAs and home health aides as needed
• Documenting progress and participating in discharge planning

What We're Looking For

Required:

• Active Ohio PT license
• Degree in Physical Therapy (Bachelor’s, Master’s, or Doctorate)
• CPR certification
• Valid driver’s license, reliable transportation, and auto insurance
• Recent TB test

Preferred:

• 2+ years of PT experience
• Home health or community-based care experience
• Strong communication and organizational skills
• Ability to build rapport and lead with empathy
