About the Role:

Communi Care Rehab, a member of the Communi Care Family of Companies, is seeking a Part Time Physical Therapist (PT) to join our team at Wood Glen Alzheimer's Community. Qualifications:

• Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program
• Valid or pending State Physical Therapy License

Responsibilities:

• Evaluate, review, and screen patients/residents’ medical charts and develop treatment plans based on physician orders and patient needs
• Implement treatment plans through direct therapy and educate team members, including supervising PTAs
• Participate in interdisciplinary meetings, relay clinical findings, and make recommendations; conduct staff and family training as appropriate
