Full‑time Physical Therapist Case Manager
Anticipated territory:
Yolo County, Davis*
- Perform an evaluation at the patient’s home and develop a care plan.
- Work with PTAs to follow the care plan and help patients with mobility, movement, and regaining independence.
- Create detailed treatment and care plans for each patient.
- Communicate and coordinate with patients’ physicians to ensure a quality level of care.
- Evaluate and report patients’ progress to the physician.
- Concierge service: assistance with documentation and administrative tasks.
- Flexible work schedules: create your own schedule and hours.
- Targeted territories: zip‑code based assignments to reduce driving time.
- Autonomy: no clocking in/out.

- Mileage/vehicle reimbursement: $0.70 per mile between visits.

- Pre‑scheduling an interview requires completion of a computer literacy and competency assessment.
- Current unrestricted California Physical Therapist license.
- Current BLS/CPR certification.
- Valid California driver’s license.
- Knowledge of local community resources.
- Understanding of individual development and human behavior as it relates to illness/injury; knowledge of cultural influences health care.
- Knowledge of state and federal home‑ regulatory guidelines preferred.
- Experience in home health care highly preferred.
- Experience with high‑acuity patients a plus.
- Experience working with an interdisciplinary team highly desired.
- Case management fundamentals highly desired.
- Self‑directed, empathetic, critical thinker.
- Outstanding interpersonal skills, leadership skills, organized.

Wage range Physical Therapist, Level 1: $–$135,.00 plus $10,000 annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements. 21st Century offers generous bonus incentive plans, comprehensive health benefits and 401(k) (up to 4% match). Majority of PT Case Managers are hired as PTIs; upon meeting leadership criteria they may be promoted to PTII with a minimum salary of $135,000 plus $20,000 bonus, paid quarterly.
