We are seeking an experienced Practice Nurse to join our well established nursing team. The successful candidate will be responsible for the delivery of practice nursing services, working as part of the practice's multidisciplinary team and providing care within their scope of practice to the registered patient population.

The Practice Nurse will have responsibility for a range of clinical areas, including health promotion, chronic disease management, disease prevention, and will also support the management team in the review and development of clinical policies and procedures.

This post is available on a full‑time basis of 37.5 hours per week; however, applications for two part‑time posts will also be considered.

Main duties of the job
  • Develop, implement and embed health promotion and well‑being programmes
  • Implement and evaluate individual treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects on the patients' health
  • Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
  • Request pathology services as necessary
  • Process pathology results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure read codes are used effectively
  • Maintain chronic disease registers
  • Develop, implement and embed well‑woman clinics
  • Develop, implement and embed well‑man clinics
  • Chaperone patients where necessary
  • Prioritise health issues and intervene appropriately
  • Recognise, assess and refer patients presenting with mental health needs
  • Implement vaccination programmes for adults and children
  • Support patients in the use of their prescribed medicines or over‑the‑counter medicines (within own scope of practice)
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  • Support the clinical team with all safeguarding matters, in accordance with local and national policies
  • Deliver opportunistic health promotion where appropriate

Darwen Healthlink is a busy, friendly and expanding practice with 4 GP Partners providing a caring service to a population of approximately 15,000 registered patients. To support our future plans and service development, we are seeking one full‑time or two part‑time Practice Nurses to join our clinical team.

Our clinical team consists of

  • 4 GP Partners
  • 1 Advanced Nurse Practitioner
  • 4 Practice Nurses

They are supported by a Practice Manager, Office Supervisor, Practice Operations Assistant and an efficient and professional administrative team, which has received training in care navigation and workflow optimisation.
