• Deliver exceptional person-centred care with dignity and respect
• Co-create care plans that honour individuality

• Level 2 Diploma in Health and Social Care or equivalent experience
• Proven ability to support individuals with empathy and expertise
• Clear, confident communicator with strong written and spoken English
