Complex care nurse

This is a full-time, on-site Complex Care Nurse role located in Dartmouth. The Complex Care Nurse will be responsible for providing advanced nursing care to patients with chronic or long-term health conditions. Key responsibilities include conducting patient assessments, developing and implementing care plans, administering medications, monitoring patient outcomes, and collaborating with a multidisciplinary healthcare team. The role involves providing compassionate care while ensuring patient safety and promoting health improvements.

Strong communication with patients and families, as well as adherence to healthcare regulations and standards, will be integral to the role.

• Comprehensive clinical skills in patient assessments, care planning, and monitoring chronic or complex health conditions
• Proficiency in administering medications, monitoring medical equipment, and responding effectively to patient needs
• Strong interpersonal and communication skills to work collaboratively with patients, families, and multidisciplinary teams
• Knowledge of healthcare regulations, infection control protocols, and best practices in nursing care
• Empathy, problem-solving abilities, and a commitment to patient-centered care
• Current nursing license and certifications, such as Basic Life Support (BLS) or Advanced Cardiovascular Life Support (ACLS)
• Experience in complex care or chronic disease management is highly desirable
