What you will do
  • Conduct assessments under the Care Act 2014 for individuals within the Adult and Older People’s Mental Health team.
  • Manage a case load, ensuring timely and thorough completion of annual reviews.
  • Participate in the office duty rota and provide crucial support during crisis situations by engaging in joint working.
  • Collaborate with Secondary Mental Health Services using an MDT approach to maintain and enhance individuals’ mental well-being.
  • Develop and implement care plans, working directly with service users and their support networks to achieve optimum outcomes.
