Responsible for contributing to the nursing assessment, patient admissions, participating in the development and modification of the plan of care, implementing the plan of care, and evaluating patient responses to nursing interventions. Administer nursing care to patients under the direction and supervision of a Registered Nurse, regardless of race, religion, sex, national origin, or handicap.

• Contributes to nursing assessment by collecting, reporting, and recording objective and subjective data.
• Observes and reports changes in patient condition and signs and symptoms of deviations from normal health status.
• Participates in the development and modification of the Nursing Care Plan by providing data and using it to plan and prioritize work.
• Documents the involvement of patient/family/significant other in the medical record.
• Identifies appropriate nursing interventions to maintain comfort and support human functions.
• Assists with discharge planning and performs nursing procedures for patient comfort and well‑being.
• Assists patients with ambulation and with special procedures such as prescription exercises, turning, massages, backrubs, and repositioning.
• Monitors and records vital signs, neurological and neurovascular checks, and reports abnormalities.
• Performs 12‑lead EKG, applies electrodes for cardiac monitoring, and documents results.
• Interprets cardiac rhythm including sinus rhythm, bradycardia, tachycardia, atrial fibrillation, atrial flutter, ventricular flutter, and ventricular fibrillation.
• Prepares patients for meals, assists or feeds as needed, verifies placement of feeding tubes, and administers tube feedings.
• Monitors oxygen therapy for safety and effectiveness.
• Assists with nursing care procedures such as suctioning, intubation, extubation, and peritoneal dialysis as appropriate.
• Provides elimination care—including enemas, irrigations, ostomy care, and urinary catheterizations.
• Collects, labels, and delivers specimens; monitors and records fluid balance.
• Assists with IV therapy in accordance with hospital policy and Alabama Board of Nursing scope of practice guidelines; performs venipunctures and I.V. site care.
• Administers medications per policy, documents orders, observes for reactions, and reports as needed.
• Applies treatments, dressings, or corrective devices as ordered.
• Ensures restraint orders are written and renewed; assists staff in identifying restraint alternatives.
• Monitors blood glucose via glucometer and records values.
• Transcribes physician orders onto MAR Kardex and implements verified orders.
• Documents nursing interventions and patient responses, including vital signs, medications, treatments, and wound care.
• Explains interventions to patients and responsible persons, providing education as appropriate.
• Performs phlebotomy and collects lab specimens per standards.
• Monitors skin integrity, reports abnormalities, and assists with wound care.
• Assesses pain using a 1‑10 scale or other methods and intervenes promptly.
• Performs other duties as assigned.

Must be currently licensed as an LPN by the State of Alabama.

Minimum one year experience; hospital experience strongly preferred.
