Base Pay Range: $70,000.00 – $80,000.00 per year

Founded in 2017, Vitae Health Systems is one of the largest providers of internal medicine and behavioral health services to the residents of Skilled Nursing and Assisted Living Facilities. With headquarters in Chicago, IL, we currently operate in over 10 states and are rapidly expanding throughout the USA.

As we continue to grow, we are looking for part‑time and full‑time licensed professional counselors to work with the geriatric population in a Skilled Nursing and/or Assisted Living setting.

• A dedicated patient base and consistent referral flow.
• Collaboration with the skilled‑nursing facility’s integrated care team and Vitae’s clinical team (adult nurse practitioners, psychiatric nurse practitioners, psychologists, LCSWs).
• A balanced work‑life schedule with flexible hours; no nights, weekends, or off‑hours required.
• Malpractice insurance coverage of $1 million/$3 million provided by Vitae.

• Hold an active and unrestricted LPC in the state.
• Hold a completed master’s in counseling.
