Overview

The CRIS APS Caseworker is responsible for investigating reports of abuse, neglect and exploitation in Vermilion County involving adults with disabilities 18 and older and adults 60+ in accordance with the Illinois Adult Protective Services Act. The caseworker will assist individuals who maybe in need of services, through communication with, family members, physicians, hospital staff, law enforcement, State's Attorney's office and the judicial system.

The caseworker will be responsible for assessment of potential clients to determine need for services and will provide resources and referrals.

Responsibilities

Complete assessments of potential clients to determine need for services. Create an intake, case notes and case plan based on individual needs. Maintain accurate, complete and timely documentation on all client activities and services provided, through intakes forms, case plans and case notes. Make appropriate referrals to other agencies and to programs administered by CRIS to meet the needs of clients.

Must have dependable transportation and be able to travel to and access client’s homes. To include but not limited to: stairs, ramps, fences, being able to stand for an unknowing amount of time. Ability to communicate clearly, both orally and in writing, with the older population, age 60 and over, and/or disabled individuals, age 18-59, who may have hearing and/or speech impediments, as well as, program employees, community organizations/agencies, professionals and community members.

Must be able to clearly communicate face to face and on the telephone. Must be able to organize and prioritize caseload to meet required standards of the Adult Protective Services standards set by the Older American’s Act, Adult Protective Services Act and IDOA. Will be able and willing to advocate/support each client based on individual needs. Must be able to perform face to face assessments of alleged victims as well as alleged abusers.

Must be able to determine victim/caregiver needs and have knowledge of appropriate intervention services to provide protection and to advocate/support client to the extent the client wants assistance and to be able to communicate the need to petition the court should the client lack capacity to self-determination. Provide follow-up contact and outreach services as needed to ensure that the client stays in the mainstream of society and to prevent reclusiveness and /or depression.

Maintain a working network with the client's physician, family members, Judicial System, State’s Attorney’s Office, Women’s Shelter, Sexual Assault advocates, Law Enforcement, Attorneys etc., to ensure services are provided to clients. Become and stay familiar with services, resources and programs offered in the community to be able to assist clients. Provide information, support and assistance to clients in need of Orders of Protections.

Work closely with other CRIS administrative and program staff in accomplishing all program and agency goals through community education, services and by participating in joint program activities and events. Complete monthly and quarterly reports as stipulated in grant requirements. Actively seek and find persons aged 60 and over as well as individuals age 18-59 with a disability in Vermilion County who maybe in need of services, through communication with law enforcement, State’s Attorney’s office and the judicial system.

You are a Mandated Reporter when in a professional capacity when the person is unable to create a report on their own behalf. Ensure intakes are completed for all suspected abuse, neglect, financial exploitation and self-neglect issues…
