Licensed Practical Nurse (LPN)

Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Hathorne Hill Rehabilitation and Healthcare Center in Danvers, Massachusetts. As an LPN, you will play a vital role in providing high-quality care to our residents, working under the supervision of our RN Supervisor/ADNS. Responsibilities
  • Administers medication and treatments according to physician's orders
  • Assists with ordering medications
  • Transcribes, documents, and implements physician's orders
  • Collects data to assist in nursing assessments during each shift
  • Gives daily shift report to CNAs assigned to the unit
  • Reviews written daily assignments for each resident on assigned unit
  • Assures assignments are completed before the end of the shift
  • Provides care to residents, including making rounds at least once every two hours
  • Collaborates with Care Plan Coordinator to develop and maintain nursing care plans
  • Attends and participates in interdisciplinary resident care conferences
  • Assists attending physician with rounds and special procedures as needed
  • Participates in yearly written evaluations of nursing assistants
  • Assists Staff Development with orientation and on-the-job training for new nursing personnel
  • Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants
  • Notifies nursing supervisor, resident's physician, and family of a change in the resident's condition or an accident/incident involving the resident
  • Assumes any duties assigned by the RN Supervisor/ADNS to provide optimal quality resident care
Requirements
  • Licensed Practical Nurse licensed in the State of Connecticut
  • Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred
  • CPR certified
  • Ability to read, write, and comprehend English; ability to follow oral and written instructions
  • Fluent knowledge of the MS Office suite and other office equipment
  • Ability to work hours as scheduled based on the requirements of the position assignment
  • Physically able to move intermittently throughout the day, proper body mechanics required
  • Ability to lift a minimum of 50 lbs.

  • Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately
