Licensed Practical Nurse (LPN)Job Summary
We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Hathorne Hill Rehabilitation and Healthcare Center in Danvers, Massachusetts. Responsibilities- Administers medication and treatments according to physician's orders
- Assists with ordering medications
- Transcribes, documents, and implements physician's orders
- Collects data to assist in nursing assessments during each shift
- Gives daily shift report to CNAs assigned to the unit
- Reviews written daily assignments for each resident on assigned unit
- Assures assignments are completed before the end of the shift
- Provides care to residents, including making rounds at least once every two hours
- Collaborates with Care Plan Coordinator to develop and maintain nursing care plans
- Attends and participates in interdisciplinary resident care conferences
- Assists attending physician with rounds and special procedures as needed
- Participates in yearly written evaluations of nursing assistants
- Assists Staff Development with orientation and on-the-job training for new nursing personnel
- Collaborates with nursing supervisor in documenting disciplinary concerns for nursing assistants
- Notifies nursing supervisor, resident's physician, and family of a change in the resident's condition or an accident/incident involving the resident
- Assumes any duties assigned by the RN Supervisor/ADNS to provide optimal quality resident care
- Licensed Practical Nurse licensed in the State of Connecticut
- Minimum of one year of education or experience in geriatric nursing or rehabilitation preferred
- CPR certified
- Ability to read, write, and comprehend English; ability to follow oral and written instructions
- Fluent knowledge of the MS Office suite and other office equipment
- Ability to work hours as scheduled based on the requirements of the position assignment
- Physically able to move intermittently throughout the day, proper body mechanics required
- Ability to lift a minimum of 50 lbs.

- Must be able to see and hear or use prosthetics/equipment that will enable these senses to function adequately
