RN Case Manager

Summary:

Achieve the physical, mental, and social well-being of the client within agency policy. Support utilization of medical resources through clinical expertise. Monitor clinical, functional, and financial outcomes. Take responsibility for ongoing interdisciplinary assessment and development of the individualized care plan in partnership with the patient, their representative (if any), and caregiver. Communicate with the physician responsible for the home health plan of care and other healthcare practitioners as appropriate.

Responsibilities:

• Assess physical, emotional, socio-economic, and environmental needs of patients and their families.
• Provide services ordered by the physician as indicated in the care plan. Communicate with all involved physicians and healthcare practitioners.
• Develop, implement, and evaluate the care plan collaboratively with the patient, caregiver, family, and physician(s).
• Coordinate services with the patient, family, and others involved. Offer counseling and education to patients and caregivers.
• Regularly evaluate patient progress, act promptly on changes in condition, and terminate care when goals are met or according to agency policy.
• Participate in quality assurance, performance improvement programs, and agency-sponsored training.
• Identify additional home health service needs and refer when appropriate.
• Supervise services provided by homemakers/home health aides, ensuring compliance with state, federal, and Joint Commission standards, and submit evaluations.
• Complete all compliance responsibilities, including clinical notes and training attendance.
• Perform other duties as assigned.

Education:

Associate's Degree in Nursing (or equivalent). RN diploma with over 3 years of relevant experience considered in lieu of degree.

Other Requirements:
Strong research skills, clear communication, knowledge of systems integration. Minimum one year of experience. Preferred:
Bachelor's Degree in Nursing and experience in a qualified home health agency.

Credentials: RN license required.
